How to enter & win:
The survey is very simple. It will take you a few minutes to complete it. Just retain your receipt and visit the website within 14 days after the Visit Date printed on the front of your receipt. Enter the store #, TC # and Visit Date located on the front of your receipt. Then follow the instructions to answer some questions about your shopping experience. At the end, you will be automatically entered into the prize draw.
