How to enter:
First, visit www.Nike.com/storesurvey in a standard web browser, and enter the the ID number provided below the survey URL on your Nike receipt. Click the “begin Survey” button at the bottom to start the survey.
Then follow the instructions to answer the questions about your store experience.
At the end of the survey, you will be asked to provide your profile. After you submit, you will be entered into the sweepstakes. There is a prize awarded each month. You must be 14 years or older to take part in the sweepstakes.
