What is Data Match?

The law requires the Internal Revenue Service (1RS), the Social Security Administration (SSA), and CMS to share certain information that each agency has about Medicare beneficiaries and their spouses. The process for sharing this information is called the Data Match.

How to Submit your Data Match Questionnaire:

Visit this link using a standard Web browser, accept the login warning, then you can go to the login page. If you’re the first time user, you should register first with the Employer Identification Number (EIN) and Personal Identification Number (PIN) printed above the Data Match file you received. If you have already registered, just login with your ID and password.
After login, you will have access to online documentation and a detailed user guide that provides a complete set of instructions. Follow the instructions and complete the Questionnaire.

  2. Are you kidding. How the heck am I suppose to remember something a year ago I cannot even get past login in it doesnt let me sign in to find out what my password was is or anything HELP

  3. My husband and I are both employed by A-1 Awards, Inc. I filled out company information on line but it seems very complicated. We now both receive SS benefits. We carry our own Medicare suplement insurance to Medicare. Our employees are all covered on an insurance plan that we have for them. No one else qualifies for SS or Medicare. Do I still need to complete this information on line or can I recieve a form by mail?

    I also tried to repond to this several times and can’t get through using explorer.
    Why did we receive a letter if there is no way to send in the information you say that you need from us.

    I too have tried to complete thisand am unable to do so. I guess I will wait until someone contacts be. My employer information is there but it will not let me get past that page. What the heck?

    I tried to get in DATA MATCH QUESTIONNAIRE but we could not. CMS needs to review this program and help businesses answer the questions. March 18, 2013
